What You’ll Need

To request replacements, you’ll need to complete an Application for Replacement Plates, Stickers, and Documents (REG 156).

Depending on your situation, additional items may be required, such as:

• A valid California Driver’s License or ID card
  
  

• If you’re from out of state or outside the U.S., your driver’s license or ID plus a secondary photo ID
